DLIR is pleased to offer the following options for telephone appointments.

• Unemployment Insurance Adjudication –
• Unemployment Insurance Claims Offices –
• Unemployment Pandemic Unemployment Assistance –
• Unemployment Employment Services Office –

For telephone appointments, you will be contacted during your appointment time. Please be aware that the caller identification may appear as a restricted number.

To make the appointment:

• Choose an appointment date/time. Please note that appointments are available only two weeks in advance.
• Complete the form with your personal information.
• Upon submission of the form, you will receive a confirmation email.

Some important notes:

• If your claim has been pending for less than 3 weeks your appointment will be cancelled.
• Appointments are restricted to the registered person on the appointment schedule.
• You must schedule an appointment with the section that can service your claim. For example, if your request for payments is pending because of an overpayment or job separation, or eligibility issue, then your appointment would fall under the adjudication section.
• Your information will be reviewed and approved for a telephone appointment. Appointments will be cancelled if you register for an appointment under the incorrect section.
• Repeated violations of rules and/or creating multiple appointments, continually creating appointments in the incorrect section will result in the in-ability to make future appointments.
