Incident Reporting Requirements

Pursuant to §12-52.1, HAR, report to HIOSH office of the following:

within 8 hours after the death of an employee from work-related incident; or
within 24 hours of:
- inpatient hospitalization of an employee,
- employee’s amputation,
- employee’s loss of an eye as a result of a work-related incident, or
- property damage in excess of $25,000 as a result of work-related incident.
