Our Service Provider Partners are an integral part of The Emergency Food Assistance Program (TEFAP). The Office of Community Services (OCS) has created standardized State of Hawaii TEFAP forms to assist our partners. In accordance with “The Emergency Food Assistance Program Distribution Plan for the State of Hawaii”, OCS will post and update these forms on our website for easy access.
